I bleed from everytime i go number 2 and im wipeing theres blood?

It hurts when I wipe. And whn I just apply the toilet paper to it... it has blood on it and could it be because when I wipe I crumble the toilet paper? Also it don't bugg and I don't feel sick what could it be??
